Kolobova, Chair of Obstetrics and Gynecology, Advanced Training and Professional Development Center, State Educational Institution, Nizhny Novgorod State Medical Academy, Department of Health and Social Development Summary. The article describes the status of the total antioxidant and free radical activity of the blood in women suffering from pregnancy noncarrying of infectious genesis. It is noted that the electric pulse therapy applied in the combination treatment of the pregnants suffering from urogenital infections provides more significant improvement of the parameters of chemiluminescent light emission of the blood serum as compared with the traditional treatment. ^ SCENAR-therapy, electric pulse therapy, pregnancy, noncarrying of pregnancy, urinogenital infections, clamidiosis, mycoplasmosis, ureaplasmosis, cytomegalovirus, herpes simplex virus, free radical activity, antioxidant activity. Actuality. The reseaches performed within last years stated the important role which the processes of lipid peroxidation and body anioxidant protection play in the pathogenesis of pregnancy noncarrying of infectious genesis, disorders of embryo- and fetogenesis. It is known that the disorder of the structure, function and increase of cell membranes permeability whose key mechanism includes lipid peroxidation processes [1-10] are of great concern in the etiology of the inflammatory processes in the body. ^ To compare the changes of free radical activity of the blood serum in pregnants suffering from noncarrying of infectious genesis when applying the traditional methods to treat the urogenital infections and when including SCENAR-therapy (electric pulse therapy with high-amplitude short-wave low-frequency electric signals) in the combination treatment. ^ 30 pregnants suffering from noncarrying of infectious genesis were examined. All the 10-12, 22-24 and 36-38 weeks pregnants were examined to reveal the urinogenital infections (Chlamidia, mycoplasmal, cytomegaloviral and herpetic infection). All the patients with the carrier state of the infectious agents were divided into 2 groups compared according to the age, social status, previous gynaecological and extragenital diseases. The 16-18 weeks pregnants included in the control group (15 persons) were prescribed to treat the urogenital infections applying the traditional methods (Josamycin was applied by 500 gr three times per day for 10 days at Chlamidia and mycoplasmal infection, Pimafucin - by 400 mg twice per day for 10 days; at cytomegalovirus infection aggravation, the human normal immunoglobulin - by 25 ml for 200 ml of normal saline solution, IV infusion, every other day, total - 3 times during the treatment course (the repeated courses to be performed according to the indications at 23-24 weeks and 10-14 before the childbirth); at primary herpetic infection or chronic process aggravation, zovirax was applied by 200 mg 5 times per day for 5 days; immunocorrective drugs were applied together with etiotropic treatment: viferon -1 per rectum 2 times per day for 10 days). The 16-18 weeks pregnants included in the main group (15 persons) received the traditional treatment combined with electric pulse therapy provided by SCENAR-1 NT (OKB RITM, Taganrog) with using pulse bipolar current without constant component in the form of the pulses with the frequency from 10 up to 350 Hz activating the oscillating frequency mode within the limits 30-120 Hz with formation of the pulses in the bursts from 2 up to 8 and the repetition rate from 540 Hz up to 4,5 kHz. Application of SCENAR-therapy in the practical health service was approved at the session of the Section on Traditional Treatment Methods of the Scientific Council of RF Health Ministry (Minutes №3 dated December 20, 2000). The experiment performed on the laboratory animal proved that SCENAR electric signals have not embryotoxic and teratogenic properties and this fact allowed to apply this method in pregnant women. SCENAR-therapy was applied to the pregnant patients according to the permission issued by the local ethical committee at State Health Care Institution “Nizhny Novgorod Regional Clinical Hospital named after N.А. Semashko” (Minutes №7 of the session of the committee on the ethics at State Health Care Institution “Nizhny Novgorod Regional Clinical Hospital named after N.А. Semashko” dated October 11, 2005). During the session, the action was provided on biologically active zones of the skin surface by the electrode built in the device housing. SCENAR-therapy course included 10 daily sessions with duration of 35-40 minutes each. The general technique of action was applied with the pulse frequency of 90,7 Hz. This technique included treatement of the vertebral, paravertebral lines and six points of trifacial nerve output on the face, cervical-collar zone, liver projection areas. The small pelvis area was not treated. Our works demonstrated that SCENAR-therapy application resulted in more significant reduction of pregnancy noncarrying cases, inflammatory processes in the afterbirth and embryo infection as compared with traditional treatment of urogenital infections [4]. According to the objective of this research performed in 10-12, 22-24 and 36-38 weeks of pregnancy, the examination of free radical activity of the blood serum applying the induced chemicoluminescence method was performed to all the patients included in the control and main groups using the biochemiluminometer БХЛ-06М (BKhL-06M, Nizhny Novgorod) interfaced with IВМ computer. The method applying induced hydric dioxide and ferric sulphate chemicoluminescence is based on the fact that the catalytic decomposition of the hydric dioxide with metal ions with variable valence - ferrous iron happens in the given system: ROOH + Fe 2+ → RO• + OH‾ + Fe 3+ (Phenton reaction). The formed free radicals RO• and OH‾ enter into the reaction of free-radical oxidation activation in the biotic substrate that results in formation of unstable tetroxide decomposed with light quantum discharge registered on BKhL-06М. The intensity of the light emission process is affected by the complete complex of compounds which may provide oxidant and prooxidant actions [11]. Imax indexes are considered to be informative indexes – maximum intensity of the light emission of the sample being examined and measured in mV. Imax reflects the free radical activity of the sample. S index– light sum of chemicoluminescence within the specified period which is reciprocally proportional to the antioxidant activity (АОА) of the sample. АОА – summary physical – chemical value defining the ability of this substrate to short-stop the oxidation reactions. It depends on the relative quantity and physical-chemical parameters of each bioantioxidant included in the mix being analyzed, their mutual influence one on the other, on the available substances which may reinforce or suppress the bioantioxidants action [12]. The information about the parameters of the chemiluminescent light emission of the blood serum in healthy pregnants was obtained at the Chair of Biology of Nizhny Novgorod State Medical Academy. ^ The parameters of the chemiluminescent light emission of the blood serum in patients suffering from pregnancy noncarrying of infectious genesis are given in Table. Table 1 Table. Parameters of the chemiluminescent light emission of the blood serum in pregnants suffering from noncarrying of infectious genesis, М±m
Note: * р =0,00001; р=0,00003 – asterisk marks the data whose values are reliable relative to the control group According to the data given in Table, at all 10-12 weeks pregnants of the control and main groups, the complete antioxidant activity was marked to be decreased by 1,54 and free radical intensity was marked to be increased by 1,51. According to Т.Yu. Pestrikova and V.F.Grigoryev [9], I.V. Pak [8], Т.S. Kachalina and N.Yu. Katkova [5], it may prove the metabolic disorders against the background of the urogenital infections activation and threatened miscarriage. Then, when examining 22-24 weeks pregnants, we marked that the decrase of the inflammatory process activity against the background of the performed therapy was accompanied with insignificant increase of the total antioxidant activity of the blood for 17% and decrease of the free radical oxidation intensity for 19,9% in the control group while these values in the main group were as follows– for 23,0% (р=0,00003) and 25,6% accordingly (р=0,00001) At 36-38 weeks of pregnancy, as compared with the parameters of chemiluminescent light emission at 10-12 weeks, we revealed that, the following increase of the total antioxodant activity of the blood for 26,5% and decrase of the free radical oxidation intensity for 23,8% happen in the women of the control group while these values in the main group were as follows – for 47,9% (р=0,00003) and 29% (р=0,00001) accordingly that may suggest more effective correction of the metabolic disorders when applying SCENAR-therapy in the combination treatment of pregnancy noncarrying of infectious genesis. So, against the background of the performed treatement of all the patients of both groups being examined, by 36-38 weeks of gestation, the total antioxidant activity of the blood serum increased due to the decrease of the intensity of free radical oxidation processes. Only SCENAR-therapy application in the complex treatment of the pregnancy noncarrying of infectious genesis provided more considerable improvement of the indexes of the chemiluminescent light emission of the blood serum as compared with the traditional treatment. Conclusion. SCENAR-therapy application in the combination treatment of the pregnancy noncarrying of infectious genesis increases the total antioxidant activity of the blood serum for 47,9% and decreases the free radical intensity for 29% as compared with the traditional treatment.
